Child Self-Assessment

Poor appetite or overeating
Insomnia or hypersomnia
Low energy or fatigue
Low self-esteem
Poor concentration or difficulty making decisions
Feelings of hopelessness
Never without symptoms for more than 2 months over a 1 year period
